- Connecting
Care is a local, pioneering initiative to connect health and care records
across the region.
- The
Connecting Care summary is included in the Bristol Health Data Source Inventory
v0.6, a prototype dataset that describes a snapshot of local health and
care related data in and around Bristol.
- The
Connecting Care data set changes regularly and the information held here
is purely an indicative snapshot.
The
HDSI (v0.6) is a prototype partnership ‘information asset register’ (*1) drawn
from local organisations during late 2016 and early 2017, and has been collated
from the organisations that contribute to Bristol Health Partners (*2),
selected partners in our local Sustainability and Transformation Partnership
(*3) and key health informatics and research initiatives (*4). The HDSI
contains no personal data.
The
HDSI provides an overview of data sets, data systems and data sources on
people’s health; wider determinants of health and some health service data.
It
is non exhaustive and presented as an example of an information asset register.
Data items within are indicative rather than an up-to-date picture of current
assets.

The Patient data in the Health Data Sources referred to in the inventory is
shared for direct clinical care and for approved official administrative use,
health research and other regulated uses. Access requests for the data will be
fielded to the appropriate health research organisation such as the Health
Research Authority and NHS Digital and the ICO and national data guardian, all
of which provide guidance on how to apply for, and conduct research using
patient data.
